Queries on Survey Form

Hello All,

I have created a survey form have some queries on the same

1) To one of my queries I have selected the checkbox option, however the answers are aligned one below the other, if I need to align them on the same line how can I perform the same screen shot attached.

2) If I need to make Query No 2 as Query No 1 how do I do the same?

3) If I need to convery my form to PDF Attachment what settings do I need to make so that I need to take a print for the same.
